The Athlete Waiver and Release and the Code of Conduct are separate documents. Complete one, review it, sign, and download the PDF. Then do the same for the other. Email each file to Joella. These pages do not email the files for you.

Read the full agreement. The athlete signs, and a parent or guardian signs when the document asks. The Get Fit Families signature stays outstanding.
Complete the waiverCode of Conduct
Initial each of the 16 statements. The player and a parent or guardian then sign.
Complete the code of conductCurrent files
The current waiver file and code of conduct file remain available.
What happens after the download?
The PDF stays on your device until you email it. Attach the downloaded file in your email to Joella. A link in the message does not include the PDF. These pages do not email it for you, and the download is not marked as received.
